Patient Participation Group

The Practice is looking for new people and patients to join our Patient Participation Group as over the last few years we have lost a few of our members. If you would like to become a member of the group and give a little bit of time to the practice by attending meetings and meeting with outside organisations, please ask at reception for details or ask to speak to our Patient Engagement Lead or Practice Manager they are happy to help at all times.

The Patient Participation Group meets up to 4 times a year at the Practice on different days and times to accommodate all the members. Patients are also invited to Patient Network meetings details of these dates are sent out to the group and displayed in the reception area of the practice. Help with costs for transport can be arranged for patients who attend these meetings. we would like to encourage involvement to help us move forward with change and technology.

Patient Engagment Lead

The Practice Patient Engagement Lead is Razia Bibi and she is here to help with any queries the patients may have. The patient engagement lead liaises with patients, outside organisations and voluntary services to guide patients in the right direction for help outside of the practice. Razia Bibi is happy to chat to patients at any time and can be contacted on the surgery number or you can all in to see her on Mondays and Wednesdays

The Practice and patients continue to communicate and meet. We encourage our patients to sign up to be part of the Patient Participation Group and meet up with the team. Moor Park Medical Practice is a small practice and have minimal rooms which are small so meeting with a big group is not always possible

The Practice works closely with our Primary Care Network 5 (PCN5) 2022 2023

The Practice and patients are working with 10 other Practices in the area, we are working as one to bring new services needed in this area to our patients.

PCN5 Practices continue to work together to deliver the booster covid vaccination programme, improving flu vaccination uptake and looking at ways to improve services for patients with Learning Difficulties. The health centre has held open days organised by PCN5 for patients with a Learning dissability and flu clinics where patients can drop in 

PCN5 are now offering appointments on an evening and at weekends at various locations for patients to attend for their Diabetes, Cervical Smear, Asthma, Bloods reviews so giving patient choice.

The patients and staff at Moor Park Medical Practice also work together with Avicenna Practice and Primrose Medical Practice on fundraising for various charities. Staff organise awareness sessions in the health centre for different medical conditions and do fundraising/ collections for charities.

The Practice encourage patient participation 

Breast, Bowel and Cervical cancer screening 2024

The Practice is working with PCN5 to improve outcomes and services for cancer screening. PCN5 have employed Care Co Ordinators to work on Cancer screening by getting patients to send samples off for screening, booking appointments and reviewing none attender lists.

The health centre are always looking at ways to promote and take part in raising awareness of breast, bowel and cervical cancer screening. The Practice has updated displays in the health centre and continued to offer appointments for Cervical Smears throughout the Covid 19 Pandemic

As well as the Practice working with PCN5 the Practice also works with Primrose Surgery, Avicenna, Bradford Moor and Dr Hamdani's practice on different projects for patients throughout the year.
