Enhanced Access
This service enables patient to be seen at other locations. These appointments include GP, ANP, Nurse, HCA, Mental Health, Physio, Bereavement counselling, Welfare Benefits help and many more. These appointments are on evenings and weekends and can be booked by telephoning or calling in at our reception. Appointments can be booked on the day or can be pre booked for patient choice so patients can see a Male or Female clinician. The service offers telephone or face to face appointments.
PCN5 work closely with Bradford Care Alliance delivering Enhanced Access Clinics across Bradford for our patients.
PCN5 - Appointments can be booked on an evening and saturday morning. These appointments are for all patients within the PCN5 area covering 10 Practices. Appointments can be pre booked at a time and date that is suitable for the pateint. The clinics cover the following, GP Appointments, Nurse, Diabetes, Asthma, Bloods/BP, and Cervical Smea, Mental Health, Berevment Counselling, Young persons mental health, Physio Appointments
Social Prescribing - PCN
PCN5 is working with the 8 Practices in this area to deliver a Social Prescribing Service. Patients can be referred to this service by any staff member from Moor Park Medical Practice The Practice have two social prescribers and they have excellent knowledge regarding local services and resources to voluntary groups. The social prescribers can gain other information for patients whatever that may be.
The service offers support to patients and families signposting them to Voluntary organisations, benefits advice, helping with isolation, loneliness, befriending, anxiousness, domestic abuse, child services support and many other services.
FLU VACCINATIONS - October 2025 - January 2026
The Practice ran flu clinics Oct 2025 to January 2026 to give patients the flu vaccination. Flu Vaccinations were encouraged at every opportunity.
FLU CLINICS - FOR OCTOBER 2026
THE CLINICS WILL COMMENCE ON THE 1ST OCTOBER 2026 AND RUN THROUGH TILL JANUARY/FEBRUARY 2027. VACCINATIONS HAVE BEEN ORDERED AND PATIENTS WILL BE INVITED TO DROP IN CLINICS/BOOKED APPOINTMENTS AND WILL BE OFFERED THE VACCINATION OPPORTUNISTICALLY AT EVERY CONTACT

Antenatal
This clinic is run by midwives. If you become pregnant, you will be referred to the midwife by the Secretary at the Practice staff may have to ask some questions to do the referral so please assist the clinicians or reception staff with this.

Child Health and Immunisation
All babies are invited for a 6 week check alongside a Postnatal Appointment for Mum babies from 8 weeks old are invited for regular check-ups and immunisations and will be seen by the health visitors.
Measles - Update 2026
Measles is a very infectious viral illness that is spread by coughs and sneezes. If you are not protected and have even passing contact with someone who has measles, the chances are that you will be infected too. If you catch measles you will probably feel very poorly and be off school or work for around 10 days. There is no treatment or cure for measles.
Symptoms of measles include fever, sore red eyes, and rash. It can be a very serious infection for some people.
Complications are more likely to occur in certain groups including people with weakened immune systems, babies under one year old and pregnant women. Complications can include chest and ear infections, fits, diarrhoea, encephalitis (infection of the brain) and brain damage. Those who develop complications may need to be admitted to hospital for treatment.
Measles is serious, around one in 5,000 individuals with measles is likely to die and since 2006, there have been 3 deaths from measles in England and Wales.
