Enhanced Access

This service enables patient to be seen at other locations. These appointments include GP, ANP, Nurse, HCA, Mental Health, Physio, Bereavement counselling, Welfare Benefits help and many more. These appointments are on evenings and weekends and can be booked by telephoning or calling in at our reception. Appointments can be booked on the day or can be pre booked for patient choice so patients can see a Male or Female clinician. The service offers telephone or face to face appointments.

ARI CLINICS - ACUTE RESPIRATORY CLINICS RAN BETWEEN 1ST JAN 2025 AND 31ST MARCH 2025 THESE CLINICS WERE WELL RECEIVED BY PATIENTS AND THE PRACTICE AS THEY OFFERRED A SPECIFIC APPOINTMENT FOR PATIENTS DEALING WITH WINTER AILMENTS/CHEST INFECTIONS. THE PRACTICE IS HOPING THESE CLINICS WILL COMMENCE AGAIN THROUGH THE WINTER OF 2026

PCN5 work closely with Bradford Care Alliance delivering Enhanced Access Clinics across Bradford for our patients.

FROM 1ST JAN 2025 TO 31ST MARCH 2025 A NEW CLINIC HAS BEEN INTORDUCED FOR PATIENTS SUFFERING SCUTE RESPIRATORY CONDITIONS. ALL PATIENTS PRESENTING WITH THESE ACUT PROBLEMS WILL BE DIRECTED TO THESE CLINICS WHICH ARE RUNNING AT DIFFERENT LOCATIONS AROUND BRADFORD.

PCN5 - Appointments can be booked on an evening and saturday morning. These appointments are for all patients within the PCN5 area covering 10 Practices. Appointments can be pre booked at a time and date that is suitable for the pateint. The clinics cover the following, GP Appointments, Nurse, Diabetes, Asthma, Bloods/BP, and Cervical Smea, Mental Health, Berevment Counselling, Young persons mental health, Physio Appointments

Social Prescribing - PCN

PCN5 is working with the 9 Practices in this area to deliver a Social Prescribing Service. Patients can be referred to this service by any staff member from Moor Park Medical Practice The Practice have two social prescribers and they have excellent knowledge regarding local services and resources to voluntary groups. The social prescribers  can gain other information for patients whatever that may be. The Practice Social prescriber initially worked through covid and tried to contact all our patients over the age of 65 to see if they had any needs other than medical, this resulted in finding isolated patients who needed some help. The Practice continues to see improvement in some of our patients with the social prescriber involved in their care. The service offers support to patients and families signposting them to Voluntary organisations, benefits advice, helping with isolation, loneliness, befriending, anxiousness, domestic abuse, child services support and many other services.

FLU VACCINATIONS - October 2024 - January 2025

The Practice ran flu clinics Sept 2024 to January 2025 to give patients the flu vaccination.  Flu Vaccinations were encouraged at every opportunity.

THE CLINICS WILL COMMENCE ON THE 1ST OCTOBER 2025 AND RUN THROUGH TILL JANUARY/FEBRUARY 2026. VACCINATIONS HAVE BEEN ORDERED AND PATIENTS WILL BE INVITED TO DROP IN CLINICS/BOOKED APPOINTMENTS AND WILL BE OFFERED THE VACCINATION OPPORTUNISTICALLY AT EVERY CONTACT

Antenatal

This clinic is run by midwives. If you become pregnant, you will be referred to the midwife by the Secretary at the Practice staff may have to ask some questions to do the referral so please assist the clinicians or reception staff with this.

Child Health and Immunisation

All babies are invited for a 6 week check alongside a Postnatal Appointment for Mum babies from 8 weeks old are invited for regular check-ups and immunisations and will be seen by the health visitors.

Measles - Update 2025

Measles is a very infectious viral illness that is spread by coughs and sneezes. If you are not protected and have even passing contact with someone who has measles, the chances are that you will be infected too. If you catch measles you will probably feel very poorly and be off school or work for around 10 days. There is no treatment or cure for measles.

Symptoms of measles include fever, sore red eyes, and rash. It can be a very serious infection for some people.

Complications are more likely to occur in certain groups including people with weakened immune systems, babies under one year old and pregnant women. Complications can include chest and ear infections, fits, diarrhoea, encephalitis (infection of the brain) and brain damage. Those who develop complications may need to be admitted to hospital for treatment.

Measles is serious, around one in 5,000 individuals with measles is likely to die and since 2006, there have been 3 deaths from measles in England and Wales.

Travel Immunisations

For patients travelling abroad, our Nursing Team offers advice and travel vaccinations if necessary.

Diabetes

Led by our Nursing team, this clinic offers advice and general health check-ups to patients diagnosed with diabetes.

Asthma / COPD

Asthma sufferers can book an appointment for this clinic which is run by our nurses who specialise in asthma care.

Weight Advice

If you would like help and advice in losing weight, please contact reception for an appointment.

Coronary Heart Disease

Clinics are run within the surgery.

Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Travelling in Europe

If you are travelling to Europe the EU has published useful information for travellers on the European website.

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

• Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
• Insurance claim forms
• Passport signing
• Prescriptions for taking medication abroad
• Private sick notes
• Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
 

FLU VACCINATIONS - September 2025 - January 2026

The Practice offers Flu Vaccinations at every opportunity throughout the months of September to March. Patients can book in to FLU clinics or normal Nurse/HCA Clinics to have the Flu Vaccination. The Practice send messages to all patients who have a mobile with authorisation for text messages and a Link to book directly in to the clinics. Patients with a home telephone number will be contacted by the Reception team and patients with no numbers will be sent a letter asking you to book the vaccination.

DIRECT BOOKING - LINKS SENT TO ALL PATIENTS REGISTERED WITH A MOBILE TELEPHONE NUMBER THE MESSAGE WILL CONTAIN A LINK AND PATIENTS CAN BOOK DIRECTLY INTO ANY OF OUR FLU CLINICS ON A DAY AND TIME THAT SUITS THEM

DROP IN FLU CLINICS - CERTAIN CLINICS ARE USED FOR DROP IN FLU VACCINATIONS, DATES AND TIMES ARE SENT OUT IN TEXT MESSAGES AND ALSO ADVERTISED AROUND THE HEALTH CENTRE

WHEN PATIENTS ATTEND THE PRACTICE FOR ANY APPOINTMENT THEY WILL BE ASKED OPPORTUNISTICALLY IF THEY WAOULD LIKE TO HAVE THE FLU VACCINATION AT THE SAME TIME

RECEPTION STAFF WILL ASK ALL PATIENTS THEY SPEAK TO OR SEE AT RECEPTION IF THEY WOULD LIKE A FLU VACCINTATIONS

SMS TEXT MESSAGES ARE SENT OUT TO ALL PATIENTS REGISTERED WITH A MOBILE TELEPHONE NUMBER ASKING THEM TO BOOK IN FOR A FLU VACCINATION

PATIENTS WHO DO NOT HAVE A MOBILE PHONE NUMBER REGISTERED AT THE PRACTICE WILL BE TELEPHONED ON THEIR HOME NUMBER BY RECEPTION STAFF TO ASK IF THEY WOULD LIKE TO BOOK A FLU VACCINATION.  IF WE ARE UNABLE TO CONTACT THE PATIENT IN THE ABOVE WAYS THEN A LETTER WILL BE SENT TO THE ADDRESS ON THE MEDICAL RECORDS

PCN5 PRIMARY CARE NETWORK CONSISTS OF 10 PRACTICES IN THIS AREA. PCN5 HAVE RUN DROP IN CLINICS FOR ALL THE PRACTICE PATIENTS TO ACCESS. 

PATIENT WITH LEARNING DIFFICULTIES HAVE BEEN INVITED TO FLU CLINICS AT THE PRACTICE AND THROUGH THE PCN. HOME VISISTS ARE ALSO CARRIED OUT FOR LD PATIENTS
