Pharmacy First

The first point of call with an ailment for patients should be the local pharmacy where they can assess you and provide you with information and medication if needed. Pharmacist are able to do a consultation for minor medical conditions. 

On the 31st January 2024 the government launched a new medical service called Pharmacy First where patients can have a consultation with a pharmacist of their choice anywhere in the country. Over 95% of Pharmacists across the country have signed up to the new government scheme and will offer the service.  Patients can now book consultations with a  Pharmacist for the following acute medical problems:


Acute Otitis Media* Ear and throat, viral
Impetigo
Infected insect bites
Shingles
Sinusitis
Sore throat
Uncomplicated urinary tract infections
Women 16-64 years

Blood Pressure Reviews and Contraception Pill

Staff at Moor Park Medical Practice will care navigate all patient enquiries and calls so patients are directed to the correct service for their ailment or problem they have. 

PRIMARY CARE NETWORKS - PCN5

PCNs have been introduced to build on existing Primary Care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for all patients

The Practice is now working collaboratively with 10 other Practices in the area to delivery improved and specific services to our patients. Moor Park Medical Practice is part of PCN5 and has helped deliver the Services most needed in our area of Bradford. 

CLICS 2024/25

Central Locality Integrated Care Service

The Practice has taken on this service as an individual Practice for 2023 and have been implementing new ways of working to deliver this service in the best possible way to our patients. When the Practice refers to the CLICS service they will be reviewed by a Community Connector and an Advanced Nurse Practitioner so covering social and medical problems. The Community connector has resources to voluntary groups and services and can gain other information for patients whatever that may be. The ANP works together with the Community Connector resolving medical issues. All these people come together with the GP's in a Multidisciplinary Team Meeting regularly

RICS 2024/25

Reducing Inequalities in City

Bradord Clinical Commissioning Group have commissioned another project called RICS which runs alongside the CLICS program for reducing health inequalities for people in Bradford. This service is working well and having a positive impact on communities and services offered to patients.

 Enhanced Access 2024/25

Moor Park Medical Practice is part of the Enhanced Access service across Bradford where patients can be seen by a GP, ANP, ACP, Physiotherapy, Nurse, Health Care Assistant appointments, Mental Health worker, Welfare benefits, Paediatric Services, Bereavement Counselling and many more. All these services run throughout the week, some evenings and weekends and all patients will be offered these appointments on a routine basis so giving excellent patient choice and services. If any patient would like to be referred to any of these services then please ask at your appointment or ask a member of the secretarial/reception team to refer you.

Reception staff will care navigate when a patient contacts the practice and will offer guidance and support and which appointments/clinician a patient requires. Appointments for these services will be booked where appropriate.

CARE NAVIGATION 2024/25

Throughout Bradford all Practices are taking part in Care Navigation which is helping patients to be directed in the right direction to the service they need to use. All the reception Staff and Practice Manager at Moor Park Medical Practice have had training for Care Navigation and will all ask the same questions when patients ring the Practice for an appointment. We would ask that patients help by answering the questions so they can be directed quickly and easily to the right service needed. Up to date Care Navigation Training has been organised by PCN5 for our 10 Practices working together and will be delivered in April 2023 for Moor Park Medical Practice Staff

Self Care and Help - Keep Well

Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition. The practice promotes self care/help and is here to help guide patients to the appropriate service and organisations.

Health Visitors

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.

Accessible Information Standard

The practice has implemented accessible information for patients and aims to make sure that disabled patients have access to information that they can understand and any communication support they may need. this includes making sure that patients can get information in different formats according to their needs.

GDPR

General Data Protection Regulation- Introduced 25th MAY 2018

The new General Data Protection Regulation has been implemented at the Practice and is there to protect patients and staff. New guidelines state that all information is confidential and in order for staff to make appointments, order prescriptions or discuss any other things relating to a patient record then they have to be speaking to the Patient or have written permission from the patient to speak to a specific person. All staff are unable to discuss anything with any other members of the family or friends. This information will be updated regularly to ensure proper implementation at the practice.

All patients at the Practice now have full online access to their medical records.  This enables patients to order repeat prescriptions online and make and cancel appointments, patients can see their medical records and results.

From 1st April 2019 the Practice has been offering 25% of appointments through online services, patients can book with a GP, ANP, Nurse and HCA.

Patients are also able to Download the NHS APP

Patient Engagment Lead

The Practice Patient Engagement Lead is Razia Bibi and she is here to help with any queries the patients may have. The patient engagement lead liaises with patients, outside organisations and voluntary services to guide patients in the right direction for help outside of the practice. Razia Bibi is happy to chat to patients at any time and can be contacted on the surgery number or you can all in to see her on Mondays and Wednesdays

Patient Participation Group

The Practice has an established Patient Participation Group but are always looking for new members to join and give their views and input, if you are interested in becoming a member please ask the reception staff and look at our patient group information on this website.

 Access

Our surgery is easily accessible to patients using a wheelchair. We also have allocated car parking for registered disabled patients, providing they have their permit on display.
